Core goals our CHW services provide:

• Community connection - Outreach and engagement to connect with those most vulnerable, meeting people in their homes, shelters or tents, meeting people where they are.

• Permanent Housing Navigation - CHWs provide a critical role in addressing homelessness by navigating clients through complex housing systems 

• Harm reduction - Community education on harm reduction practices and model for prevention of drug overdoses as well as chronic health conditions.

• Stabilization - CHWs support clients during a transitional period to ensure successful housing placement and long term health care stabilization.
